Referrals can be faxed, emailed or posted
We prefer to recieve referrals prior to the consultation so they are not forgotten on the day of the appointment
This is a private practice, therefore we do not routinely bulk bill and patients will be told this when they book. The exception is DVA clients who can access bulk biling at all clinics.
Those who require bulk billing should be directed to our clinic at Acacia Ridge
Bulk Billing:
- is offered to clients attending Acacia Ridge via Medicare
- Is offered to all DVA clients at all clinics
- patients will need to be referred for at least two visits in order to provide a complete diabetes self management assessment
- patients will need a current medicare referral, including their diabetes care plan, team care arrangement, and EPC referral form. Relevant pathology is also helpful.

Private Health Fund reimbursements
Are offered by the following funds and may be available depending on the individual level of cover

We are unable to offer HiCaps at this time due to health fund restrictions. All accounts must be submitted to the fund directly for reimbursement after payment.
